Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was completed on March 2, 2022. There were 45 children present, ranging in ages from 3 months to 5 years, with 11 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies, and nutrition. A total of 3 children's records, 3 complete new staff records, and 8 partial staff records were reviewed.

Information gathered during the inspection determined non-compliances with applicable standards or law and the violations were documented on the violation notice issued to the program.

Based on review of three children's records and verification from the director, the center did not obtain the results of a physical examination for one child within one month of enrollment.

Evidence:
1. The record for Child #2 contained the results of a physical examination dated 02/24/2022. Date of enrollment for Child #2 was 08/30/2021.
2, The director confirmed that a physical exam report was not obtained within a month of enrollment.
Plan of Correction: The center now has obtained a physical examination report for Child #2. In the future, the center will ensure that physical exam reports are obtained within in a month of enrollment.

Based record review and discussion with the director, the center did not obtain an updated health inspection within a year of the most recent one.

Evidence:
1. The most recent health inspection was dated 01/13/2021.
2. The director confirmed the last health inspection was completed on 01/13/2021.
Plan of Correction: The director has been in touch with the health inspector and has requested an inspection as soon as possible. She understands that she should call the health department about a month of when an inspection is due to schedule one.
