Inspector Notes

An unannounced inspection to investigate a self-reported incident regarding an injury sustained on site was initiated on 9/1/2023 with the local agency beginning at 10:00 a.m. to 12:15 p.m.
Staff interviews were conducted. Documentation was reviewed. Observation and inspection of the playground and play areas were conducted.

Based upon the preponderance of the evidence collected during the inspection, there was no noncompliance regarding the self-reported incident. There was other noncompliance found during the inspection that is documented on the violation notice issued to the center.

An exit interview was conducted by phone with the center director on 10/6/2023.
Please provide plan of correction for the areas of noncompliance and date to be corrected by 10/13/2023.

Based on review of documentation, the center failed to obtain documentation of a physical exam within 30 days of attendance. Evidence: The record for Child #1, first day of attendance 11/7/2022, did not contain documentation of a physical exam.

Standard 8VAC20-780-270-A
Based on observation, the center failed to ensure that all areas of the center inside and outside were maintained in a clean, safe, operable manner. Evidence: On the playground near the composite tree house, there is a large divot approximately eight inches in lenght in the ground underneath the artificial grass covering that creates a tripping hazard.
Plan of Correction: The divot was fixed on 9/13/2023

Based on review of documentation, the center failed to ensure that children?s records contained all required information. Evidence: The record for Child #1, first day of attendance 11/7/2022 did not contain written authorization/written objection for emergency medical care should an emergency occur.
