Inspector Notes
An unannounced renewal inspection was conducted beginning at 1:15 p.m. and ending at 4:30 p.m. with the director. There were 84 children in care ranging in age from 3 months through 5 years old under the care and supervision of 13 staff. Classrooms, supplies and materials were observed. Infants were observed during rest and play time. Two, three and four year olds were observed during nap. Toddlers were observed during transition time from nap and afternoon snack. The outdoor play area was observed.
Safe sleep practices were again reviewed and determined not consistent with recommended guidelines; therefore another VDSS Safe Sleep 365 flyer was provided and the provider was strongly encouraged to update their safe sleep practices and policy to be consistent with the safe sleep recommended guidelines.
Three staff records and four children?s records were reviewed. Five medications and authorizations were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations
2Based on observation, the center failed to ensure that all areas and equipment of the center were maintained in clean, safe, and operable condition.
Evidence:
1. In the Toddler B classroom, in the back of the classroom on the wall, there was an approximately 1.5 inch long gash in the wall that exposed drywall and chipped, peeling paint.
2. In the School Age classroom, there were three different areas of chipped, flaking paint. On the back wall, the corner of the wall had four areas of chipped, flaking paint. On the window area, there was one large patch of missing, chipped, flaking paint on the right and there were two large patches of missing, chipped, flaking paint approximately six inches to eighteen inches long.
Evidence:
1. Child #1?s authorization for albuterol expired on 12/9/2023 and the medication was still on site. There was no current medication authorization.
2. Child #2?s authorization for albuterol expired on 1/24/2023 and the medication was still on site. There was no current medication authorization.
