Inspector Notes
An unannounced inspection was initiated on March 31, 2025 and completed on April 4, 2025 in response to a complaint received by the licensing office on March 13, 2025 relating to allegations of physical plant: clean, safe, operable, supervision, ratio and behavioral guidance. This inspection included document review, tour of the facility and interviews. Additional documentation was reviewed off-site.
The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
