Inspector Notes

An unannounced complaint inspection was initiated on 4/7/2022 and concluded on 4/28/2022 regarding a complaint received by the department on 3/28/2022. The complaint included allegations in the areas of supervision of children, behavioral guidance, and forbidden action. Two inspectors were on site on 4/7/2022 from 12:32-3:00 pm and one inspector was on site on 4/22/2022 from 9:20 am-10:15 am. The inspector(s) conducted observations and interviews were conducted both on site and remotely.
