Inspector Notes
An unannounced self report / complaint inspection was conducted on June 13, 2024 from approximately 3:40 PM to 4:45 PM. There were 113 children present in the center, ranging in ages from infant to school-age, with 19 staff supervising. The inspector reviewed compliance in the areas of administration, staffing and supervision, programming, and special care / emergencies. Interviews with staff and administrators were conducted. Video footage was reviewed. A total of one child record and one staff record was reviewed.
The preponderance of the evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, there were violations found during the inspection regarding staff to children ratio. See the violation notice for more details.
Please complete the plan of correction and the date to be corrected for each violation listed on the violation notice and return it to the department within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. 11. Staff A was alone with 20 pre-k children in room #1.
2. Administrator #2 observed video footage and reported that staff A was over the ratio (1:20) from approximately 3:38 PM to 3:41 PM in room #1.
