Inspector Notes
An unannounced monitoring inspection was initiated on 5/13/2022 and concluded on 5/23/2022
in response to a self-reported incident that the Department received on 5/10/2022 regarding inadequate supervision. The inspector was on site on 5/13/2022 from 9:44 am-11:16 am and 5/18/2022 from 9:30 am-10:00 am. Interviews with staff were completed, observations were made in the center, and documentation was reviewed. The inspector completed additional interviews remotely via telephone.
The preponderance of the evidence gathered during the investigation validates the self-reported incident of inadequate supervision. Violations were cited regarding supervision and care and protection. See the violation notice for more details.

Violations
2Evidence:
1. On 5/9/2022, child #1 (DOE: 1/10/2022) pushed open an unsecured door in the infant classroom and entered a playground area. After child #1 was observed entering the playground, Teacher #1 (DOH: 3/28/2022) noticed that the door was not secure when another child was playing by the door and she heard clicking noises. She moved the child and pulled the door completely shut, leaving child #1 locked outside. Teacher #2 (DOH: 11/6/2019) noticed that child #1 was missing and brought it to teacher #1?s attention. After searching the classroom, teacher #1 opened the back door to the playground and found child #1 outside. It was determined through video that the child was unsupervised outside for approximately 18 minutes.
2. Administration acknowledged that the door was not secure which resulted in the child?s unsupervised separation from her caregivers.
Evidence:
1. On 5/9/2022, it was determined that child #1 (13 months old) was not within sight and sound of a staff member for approximately 18 minutes when the child left the classroom and entered the playground through an unsecured door.
2. Administration acknowledged in writing on 5/17/2022 that the staff members failed to comply with safety policies by ensuring to properly supervise a child by ensuring direct line of sight, auditory awareness, or knowledge of the physical proximity of the child.
