Inspector Notes

A Subsidy Health and Safety inspection was conducted on 4/27/2023 to review supplemental health and safety requirements. There were 53 children present with 11 staff supervising.

A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection.

Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard 8VAC20-790-600-D-1
Based on a review of six (6) staff records and interview, the vendor did not ensure that two (2) staff who worked directly with children had within 90 days of the date of employment: current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care.

Evidence:
1. The records of staff #5 (date of employment: 8/31/2022) and staff #6 (date of employment: 8/31/2022) did not contain documentation of a current CPR certification.
2. The vendor acknowledged that the two staff were not certified.
Plan of Correction: cpr cards for staff #5 and 6 have been located, printed and placed in files (both expire 8/31/2024)
