Inspector Notes

An unannounced inspection was initiated on 7/27/2023 in response to complaints that were received by the licensing department on 7/14/2023 and 7/15/2023 regarding allegations in the areas of toileting procedures, behavior guidance, care and protection and forbidden action. The inspection was initiated on 7/27/2023 and completed on 8/15/2023. On 7/27/2023, the inspector was on site from approximately 8:28 am to 10:06 am. On 8/15/2023, the inspector was on site from approximately 11:00 am-2:05 pm. Interviews with staff were completed, observations were made in the center, and documentation was reviewed. Additional interviews were conducted remotely.

The preponderance of the evidence gathered during the investigation supports part of the allegation; therefore, the complaint is determined to be valid. See the violation notice for more details.

Based on observation and interview, the center did not ensure that children under the age of three years were changed on a changing table or countertop designated for changing.

Evidence:
1. In interview, staff #1 stated that the children in the toddler classroom (ages 16-24 months) were sometimes changed while standing up. Two additional staff stated that they had observed toddler aged children not being changed on a table.
2. Administration acknowledged that the children in the toddler room were not being changed on the table as required.
