During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated on 06/03/2025 and completed on 06/06/2025 in response to a complaint received by the licensing office on 05/13/2025 relating to allegations of behavioral guidance and forbidden action. The inspector reviewed one child?s record and one staff record on-site. This inspection included document review, tour of the facility, and interviews.

The preponderance of evidence gathered during the investigation supports part of the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 07/21/2025. A POC submitted after this date will not appear on the public website.

Repeat Violation:
Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts.

On 05/07/2025, Staff #1 made child #1, age six years, help unclog a toilet after toileting. Staff #1 had child #1 put gloves on to help clear material from the toilet. Staff #1 stated they saw it as a teaching opportunity for child #1.
Plan of Correction: Staff #1 was immediately placed on administrative leave until the investigation was completed. Management has conducted a retraining at our staff meeting on 5/29/25, of VDOE's regulations and LCG's policy on positive guidance and redirection. Management will continue to have ongoing monthly training on positive guidance and redirection.
