An unannounced monitoring inspection was initiated on 02/11/2026 and completed on 02/12/2026 in response to a self-report received by the licensing office on 01/12/2026 relating to a serious injury. This inspection included document review, interviews and observations.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
