Inspector Notes
An unannounced inspection was initiated on June 20, 2023, in response to a complaint that was received by the licensing department on June 6, 2023, regarding allegations of supervision. On June 20, 2023, the inspector was on site from approximately 11:05am until 12:00pm. Interviews with staff were completed, observations were made in the center, and documentation was reviewed. The preponderance of the evidence gathered during the investigation does not the allegation; therefore, the complaint is determined to be not valid. Additional violations that were not complaint related were cited and can on the facility Violation Notice.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Violations
2Evidence: At approximately 11:30 am, six children between the ages of 9 months - 23 months were observed being supervised by one staff member.
Evidence:
1. The inspector observed staff 1 changing a toddler?s diaper. Staff 1 did not wash their hands and did not wash the toddler?s hands after diapering before placing the toddler on the carpet.
2. Staff 1 proceeded to wash the hands of child #1 before diapering. Staff washed child #1?s hands wearing the same gloves from a previous diaper change.
3. The diapering surface was not sanitized between diaper changes in the toddlers.
