Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 07/29/2024 and completed on 07/29/2024. The on-site inspection began at 10:00 am and ended at 2:15pm. The inspector reviewed compliance in the areas listed above. There were sixty-three children present and eleven staff. The inspector reviewed six children?s records and six staff records on 07/29/2024. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 8/14/2024 A POC submitted after this date will not appear on the public website. Standard 8VAC20-780-160-A
Based on documentation review, center failed to ensure all staff had results of TB test before
interacting with children.

Evidence: No documentation of TB screening for Staff 2 prior to date of hire on 4/8/2024.
Plan of Correction: All staff are required to have a paperwork appointment prior to their first day of employment and TB test must be present and reviewed at that time. Staff will not be permitted to start without a valid TB test.
Standard 8VAC20-780-500-B
Based on observation, the center did not maintain a non-absorbent surface for the diapering area and did not ensure that the diapering surface shall be used only for diapering or cleaning children.

Evidence:
1. Inspector observed ripped changing table pad in Room 1. Director acknowledged the pad was ripped/exposed. Pad was replaced by director.
2. During classroom observation, inspector observed Room 2 (Infant 2) where teacher used changing table as craft table to put together an activity. Director acknowledged the teacher used table not for diaper changing due to lack of space.
Plan of Correction: Changing table pads and bench pads were ordered on the same day as the inspection. Repair of equipment will be monitored by both staff and management on a daily basis. Any item in need of repair will be report to management and repairs/replaced.

Infant and Toddler teachers were retrained on the same day of inspection on appropriate use of the changing tables.
