Inspector Notes
An unannounced inspection in response to a self-report was conducted on August 15, 2023, and concluded on September 27, 2023. The inspector was on site from approximately 3:30 PM to 4:30 PM. The self-report received by the department included allegations of forbidden actions. The inspector reviewed compliance in the areas of administration, staffing and supervision. Additional documents were sent from the provider electronically. A total of two child records and one staff record were reviewed.
The self-report is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the plan of correction and the date to be corrected for each violation listed on the violation notice and return it to the department within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures.

Violations
1Evidence:
Staff #1 did not ensure the immediate safety of each child during diapering. Staff #1 walked away from the changing table while diapering child #2 (14 months old), to intervene between child #1 and the child that was being bitten, leaving child #2 alone on the diapering surface.
