Inspector Notes

An unannounced complaint inspection was conducted in response to a complaint that was received by the department on November 9, 2023 regarding allegations of staffing and supervision and special care provisions. The investigation was initiated on December 13, 2023, and completed on January 8, 2023. The inspector was on site on December 13, 2023, from approximately 9:40 AM to 2:50 PM, conducting interviews, completing observations, and reviewing documentation. The evidence gathered during the investigation does not support the allegation; therefore, the compliant is determined to be not valid.

The inspection report was amended on February 9, 2024.
