Inspector Notes

An unannounced inspection was conducted on 5/9/24 in response to a complaint that was received by the department on 04/24/24. The inspection was regarding staffing & supervision, nutrition and behavior guidance. Two licensing staff were on site on 05/09/2024 from approximately 9:45am until 12:15pm conducting interviews, completing observations. On 05/17/2024, the inspector was on site with a local agency conducting additional interviews from 4pm until 5:15pm. On 05/30/2024, the licensing inspector reviewed additional documentation on site.

Staff interviews, observations, and review of documents were conducted. The preponderance of the evidence gathered during the investigation supports part of the allegations, therefore the complaint is determined to be valid.

Please complete the plan of correction and the date to be corrected for each violation listed on the violation notice and return it to the department within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures.

Based on observation and interview the center failed to maintain required ratio in one classroom.

Evidence:
1. On 05/09/2024, two licensing staff observed a grouping of 16 children ranging in ages from 17 months to 2.5 years with two supervising staff. The ratio required for the grouping of children was one staff to five children; two additional staff were required.
2. During interviews, the two staff confirmed there were a total of 16 children present. The staff reported a third staff member would be arriving. At approximately 10:05am, a third staff member arrived in the toddler classroom, and children were moved to another classroom to comply with ratio requirements.
3. The "Face-To-Name Transition Sheet" for 05/09/2024 confirmed that the classroom was out of ratio from approximately 8:40am-10:05am.
Plan of Correction: On 5/9/24, all staff were retrained on the expectation for notifying management for additional teacher support if ratio is exceeded during morning drop off time.
