During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced inspection was initiated on 09/12/2024 and completed on 09/27/2024 in response to a complaint received by the licensing office on 08/20/2024 relating to allegations of behavior guidance and ratios. The inspector reviewed one children?s records and four staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/04/2024. A POC submitted after this date will not appear on the public website.
Violations

Evidence:
1. Staff #2 was working in the toddler room on 06/17/2024. Based on interview, child #1 was
running around the room right before nap time. Staff #2 pulled child #1 by the upper arm,
sat child down on the cot and told child to lay down for a nap. Staff #2 stated when they
placed the child down, they left the crying child?s side to go grab a blanket.
2. Another staff in the room took the child to the changing table where blood was witnessed
in the mouth due to child biting their tongue. Staff #2 stated the child bit their
tongue when they sat the child down on the cot and acknowledged they sat the child down
aggressively.
Evidence:
1. On 09/12/2024, at approximately 7:52am, the licensing inspector observed 21 children in care with three staff supervising during morning group in classroom A. The youngest child in the room was 21 months old which was confirmed by staff. Two additional staff were needed to meet the 1:5 staff-to-children ratio requirement.
2. At approximately 7:56am, the ratio requirement was met when another staff arrived and took children to classroom B, leaving 11 children in care with three teachers supervising in classroom A.
Retraining with all staff on Ratio procedures- 11/12/24
Evidence:
1. At approximately 7:58am, the licensing inspector observed 12 children with one staff in the 2 year old classroom. One additional staff was needed to meet the required ratio. At approximately 8:06am, a 2nd staff entered the classroom.
2. The youngest child in the classroom was 2 years, 11 months of age confirmed by staff and documentation, requiring a 1-8 staff to child ratio.

Evidence:
1. The center's handbook states that staff must create a warm, nurturing and safe environment for the children in care.
2. On 06/17/2024, staff #2 placed child #1 down on a cot for nap time aggressively, resulting in child #1 biting their tongue.
3. Another staff in the room observed blood in the child's mouth due to biting their tongue.
Evidence:
1. Staff #2 was working in the toddler room on 06/17/2024. Based on interview, child #1 was running around the room right before nap time. Staff #2 pulled child #1 by the upper arm, sat child down on the cot and told child to lay down for a nap.
2. Staff #2 stated the child bit their tongue when they sat the child down on the cot and acknowledged they sat the child down aggressively.
Evidence:
1. On 06/17/2024, a minor injury occurred to child #1 while in care.
2. Staff acknowledged the injury but could not locate the injury report on site. No injury report was provided to the licensing inspector to view during the inspection.
