During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 09/23/2025 and completed on 09/23/2025. The on-site inspection began at 10:14am and ended at 12pm. The inspector reviewed compliance in the areas listed above. There were 58 children present and nine staff. The inspector reviewed five children?s records and five staff records on-site.

This inspection included document review, tour of the facility, interviews, observations and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Repeat Violation
Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

In the school age classroom, a coat of dust covered the ceiling vent tile. In the school age bathroom, a loose exhaust fan cover was observed hanging from the ceiling.
Plan of Correction: A work order to be placed asap.

Where playground equipment is provided, resilient surfacing shall comply with minimum safety standards.

The play structure had resilient surfacing that measured less than the required 6 inches. The slide had resilient surfacing that measured less than the required 6 inches.
Plan of Correction: Rake it asap.
Standard 8VAC20-780-500-A
Staff shall wash their hands with soap and running water before and after a diaper change.

A staff member did not wash hands after changing a child?s diaper in classroom #3.
