Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain work phone number and place of employment of each parent.
The record for child #3, child #4 and child #7 was missing work phone. The record for child #4 and child #7 was missing place of employment.

Plan of Correction: When Enrollment Paperwork is received, it will be reviewed by both the Director and the Assistant Director to assure that all sections are completed on all forms.
This is to assure that all files are complete and include place and address of employment listed for each parent or guardian and if they are not employed, list it as not acquirable. If parent or guardian work from home address, include that information. Effective immediately as of 2/22/26 with new enrollments and reviewing all currently enrolled files to assure the information is in compliance.
