Inspector Notes

An unannounced monitoring Inspection was conducted on 4/14/2022 from 11:10am to 12:00pm. There were 39 (infants to school-age) directly supervised by 6 staff. The physical plant, 5 children?s records, 1 child's medication and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, listening to stories, playing outside, and napping. Diapering and hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Based on observation, the facility did not ensure that areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition.
Evidence: observed on the Pre-K and SACC playground, loose boards on fence along the rear of the playground, exposing nails/screws, accessible to children and posing a potential safety hazard.
Plan of Correction: Owners are currently scheduling a company to install new fencing on all playgrounds.
