Inspector Notes

An unannounced Monitoring Inspection was conducted today from TIME. There were 80 (Infants to 5yrs) directly supervised by 8 staff. The physical plant, 6 staff records, 5 children?s records, 5 children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed taking naps. Diapering and hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Based on observation, the facility did not ensure that the areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
Evidence: observed that the weed matting on the toddler and preschool playground was coming up and causing a potential tripping hazard.
