Inspector Notes
An unannounced monitoring inspection was conducted on 3/29/2023 from 10:30am to 12:20pm. There were 42 (4 months to 5 years) directly supervised by 7 staff. The physical plant, 5 staff records, 5 children?s records, children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, listening to stories, playing outside, eating lunch, and preparing for and taking naps. Diapering and hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. 5Evidence: Reviewed 5 staff files and found that Staff 2's last tb screening was on 11/27/2020
Evidence:
1) observed on the toddler playground, weed matting coming up through the mulch, causing a tripping hazard.
2) observed a small hole in the hall in the Twos classroom
3) observed nails protruding from the fence on the preschool playground.
Evidence: observed that the infant room had 9 children with 2 staff.
Evidence: observed in the toddler classroom, 7 children and 1 staff.
