Inspector Notes

An unannounced inspection was completed on 05/20/2025 with a local agency, in response to a complaint received by the licensing office on 05/19/2025 relating to allegations of staffing and supervision and programs. The inspector reviewed 1child record and 1 staff record. This inspection included document review, interviews, and observations.

The preponderance of evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found and are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 01/22/2026. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance. Licensing inspector entered the 2-year-old classroom and observed S1 lying down on a cot during the children?s rest period and did not appear to be alert.
Plan of Correction: Orientation and training includes child supervision per the required standards.
Standard 8VAC20-780-350-K
In addition to staff required by rest period ratios, an additional staff member shall be
always available on-site to offer immediate assistance. Upon arrival to the center, the
licensing inspector observed that all staff members were in coverage with children with
no additional staff onsite.
Plan of Correction: An additional staff member will always be available on-site to offer immediate assistance.
Standard 8VAC20-780-400-B
The center shall ensure that behavior guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior. Licensing inspector observed staff in the 3-year-old room lifting the edge of cots and rolling three awake children off the cots at the end of naptime.
Plan of Correction: Review of behavior guidance completed and ongoing training surrounding appropriate age and stage.
