Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A code compliance inspection was initiated on 06/22/2021 and concluded on 06/22/2021. The director was contacted virtually to initiate the inspection. There were 3 children present and 2 staff present. The Inspector reviewed 2 staff records and 2 children's records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined areas of non-compliance with applicable regulations or law, and violations were documented on the violation notice issued to the vendor.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Based on record review, the center did not ensure to establish and implement procedures to ensure one of two children's records are in compliance with provisions regarding the immunization of children against certain diseases.
Evidence:
The record of child #1 (start date: 10/12/19) had documentation of immunizations dated 11/8/19.
Plan of Correction: Per the provider: We are always required to get immunizations before they start. Due to extenuating circumstances around the child's records we were unable to obtain them until less than a month after enrollment.
