The inspection report was amended on 6/17/2025 to correct the inspection type.
A subsidy health and safety inspection (SHSI) was conducted in conjunction with the code compliance inspection. Subsidy standards that are not covered in code were reviewed during this SHSI.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/30/2024. A POC submitted after this date will not appear on the public website.
Violations
3The records for child #2 and child #3, who have been enrolled for over two months, did not contain documentation of a name, address or phone number for each custodial parent?s place of employment.
The record for child #6, who has a diagnosed food allergy and has been enrolled for over 2 years, did not contain an allergy action plan or any actions to be taken in an emergency situation.
The changing pad in the 2- and 3-year-old room had multiple tears on the sides making the surface absorbent.
