A subsidy health and safety inspection (SHSI) was conducted in conjunction with the code compliance inspection. Subsidy standards that are not covered in code were reviewed during this SHSI.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

For each group of children, the vendor shall maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs.

On the date of the inspection, 6 children were documented as being in attendance at the center, however, there were 15 children observed to be present during the inspection.

**Repeat Violation**
Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe, and operable condition.

In the preschool room, cracked and deteriorating wood was observed underneath the sink area cabinet. Additionally, there were approximately 16 insects observed underneath the sink area in the preschool room.

Hazardous substances shall be kept in a locked place using a safe locking method that prevents access by children.

Bleach solution was observed unlocked in a cabinet above the sinks in the preschool room, out of reach of children.
