Inspector Notes

An unannounced inspection was conducted 03/21/2024 in response to a complaint regarding nutrition and food service and supervision of children. Observations and interviews were conducted. The preponderance of evidence supports the allegations of noncompliance with standards and law. The complaint is determined to be valid.

Based on interviews the center failed to ensure that children under 10 years of age
always shall be within actual sight and sound supervision of staff.

Evidence:
A child, C1, age 16 months was found in a classroom without sight and sound supervision of staff. S2 reported that she and another staff were supervising the toddlers and 2-year-olds in the center?s inside playground. She stepped away briefly from the group to attend to another child needs. S2 did not realize that C1 had gone into the classroom out of actual sight and supervision of staff until the child?s mother came in to pick up the child and located the child.
Plan of Correction: Retraining and implementation of licensing standards on sight and supervision. Director will conduct trainings.
Standard 8VAC20-780-560-N
Based on observation and interviews the center failed to ensure that food shall be prepared, stored, and transported in a sanitary manner.

Evidence:
A child, C1 at the center used a sippy cup containing mold. Licensing inspector observed the cup and lid that were retained by the center when the parent brought the cup and mold to the center?s attention. S1 and S2 were interviewed, and they observed the spots on the cup lid. S1 and S2 reported that cleaning procedures of cups used by children at the center have been reviewed with staff.
Plan of Correction: Director will continue to provide tools and review cleaning procedures for items in classrooms.
