Inspector Notes

An unannounced monitoring inspection was completed on 05/16/2024 in response to a self-report received by the licensing office relating to special services. The inspector reviewed 1 child record and 3 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self-report, a violation that was not reported was found related to staffing and supervision and are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/25/2024. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-350-B-1
The required ratio for children birth to 16 months is 1 staff per 4 children. An incident report dated 05/07/2024 documented a brief time period where 2 of the 3 staff working in the infant room with 5 children left the classroom resulting in the classroom being out of ratio.
Plan of Correction: Staff meeting was conducted on this topic. All classrooms will remain in ratio according to age.
Standard 8VAC20-780-570-A
When a child is placed in a high chair, the protective belt shall be fastened securely. C1 was not fastened in a high chair on 05/07/2024 when she was observed standing up in a high chair. C1 lost her balance and fell hitting her head on the floor.
Plan of Correction: Training and staff meeting was conducted on this topic. Children will be fastened securely into all high chairs.
