Inspector Notes

An unannounced inspection was initiated on 09/30/2023 and completed on 10/15/2024 with local agencies, in response to a complaint received by the licensing office on 09/26/2024 relating to allegations of programs. The inspector reviewed 1 child record and 4 staff records on-site. This inspection included document, tour of the facility, interviews, and observations.

The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/25/2024. A POC submitted after this date will not appear on the public website.

The following actions or threats thereof are forbidden: roughly handling a child.
On 09/25/2024, S1 lifted C1, (age 2), off the ground by one arm to have the child get off a rocking toy. The child was observed to be favoring her arm after this incident. The center was later notified that the child had been seen at the emergency room and had sustained a break in the wrist growth plate.
Plan of Correction: Staff member terminated according to company policy. More training on topic planned for center.
