Inspector Notes

An unannounced, on-site focused inspection was completed on 01/27/2025, as a part of the provisional licensure period. The on-site inspection began at 10:20 a.m. and ended at 2:13 p.m. The inspector reviewed compliance in the areas listed above. There were 20 children present and 7 staff. The inspector reviewed 5 children?s records and 4 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 03/21/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-130-A
The center shall obtain documentation of additional immunizations once every six months for children under the age of two years. C3, who had been attending for six months did not have documentation of additional immunizations.
Plan of Correction: The center will continue asking for updated immunization records.
Standard 8VAC20-780-330-B
Playground equipment with moving parts or climbing apparatus must have appropriate fall zones with resilient surface that encompass sufficient area to include the child's trajectory in the event of a fall. The fall zones on older children?s playground measured less than 6 inches of resilient surfacing.
Plan of Correction: Playground was closed until additional resilient surfacing was added.
Standard 8VAC20-780-420-E-3
Staff shall request at least annually parent confirmation that the required information in the child's record is up to date. Up-to-date information is required to be requested from the parent for each child's record at least annually. The information in the record of C5 had not been updated in 20 months.
Plan of Correction: Updated information has been requested and added to children's records.
Center will continue to update.
Standard 8VAC20-780-430-B
Furnishing, equipment, and material shall be of an appropriate size for the children using it. Children under the age of two years old were observed using the slide and climbing apparatus located in the younger children?s play area manufactured for ages 2-3 years old.
Plan of Correction: Play slide was removed from the toddler playground.
