Inspector Notes

An unannounced, on-site renewal inspection was completed on 03/26/2025. The on-site inspection began at 10:48 a.m. and ended at 1:45 p.m. The inspector reviewed compliance in the areas listed above. There were 23 children present and 8 staff. The inspector reviewed 8 children?s records and 2 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 04/03/2025. A POC submitted after this date will not appear on the public website.

Standard 22.1-289.035-B-4
The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. S1, employed for more than 2 months, did not have documentation of requesting a sexual offender registry check from one state.
Plan of Correction: National Sex Offender Public Website search request/results now join the VA Registry search results in S1's employee file.
Standard 8VAC20-780-130-A
Repeat Violation
The center shall obtain documentation of additional immunizations once every six months for children under the age of two years. C5 and C7 who had been attending for more than six months did not have documentation of additional immunizations.
Plan of Correction: Immunization updates have been requested for children's files.
Standard 8VAC20-780-550-P
The center shall maintain written records of children?s injuries (Ouch Reports) that contains all of the required information. Injury reports dated 02/24/2025, 02/27/2025, 03/07/2025, 03/12/2025, 03/13/2025, 03/18/2025 did not document any future action to prevent recurrence of the injury. Injury reports dated 02/24/2025 and 02/27/2025 did not document the time of parental notification of the injury.
Plan of Correction: Ouch reports contain a space to document future action taken to prevent recurrence of injury. From date of violation notice, notes will be added to each report to reflect this
