Inspector Notes

An unannounced monitoring inspection was initiated on 03/31/2026 and completed on 03/31/2026 with a local agency in response to a self-report received by the licensing office on 03/20/2026 relating to programs. The inspector reviewed 1 child record, and 1 staff record on-site and electronically on 04/02/2026. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self-report, a violation that was not reported was found related to Subsidy Program Vendor Requirements for Child Day Centers and is documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 04/09/2026. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-410-1
The following actions or threats thereof are forbidden: rough handling and forcing rest. S1 was observed by S2 on 03/20/2026, via the classroom monitoring system, roughly push C1?s head back down on a rest cot with her hand when C1 appeared to be waking from a nap. S1 was then observed to fully cover the child?s head with a blanket.
Plan of Correction: The employee was immediately removed from the classroom and dismissed from employment. They had no further contact with students.

Repeat Violation
The record for S1 did not document completion of the VDOE health and safety update course due by 01/02/2026.
Plan of Correction: All employees are expected to complete trainings on time. Further instances of noncompliance will result in removal from the workplace until completion.
