Inspector Notes

An annual monitoring inspection was initiated on 02/8/2023 and concluded on 02/8/2023. The inspector was on site at the center from 1:10pm to 3:45pm. There were 74 children in care with 12 staff supervising. Interviews were held with staff throughout the inspection, and the inspector
interacted with children in each classroom when appropriate. The center?s playground, vans and all classrooms were inspected today. Seven children?s records and seven employee records were reviewed during this inspection. Information gathered during the inspection determined an area of noncompliance with applicable regulations or law, and a violation was documented on the violation notice issued to the vendor.

Based on record review and an interview, the center did not ensure one of seven staff records contained documentation of sex offender registry check results and a child abuse and neglect search request from any state in which the individual has resided in the past five years.
Evidence:
1. The record of staff #3 (employment date: 7/21/2020) did not have documentation of out of
state sex offender registry check results and a child abuse and neglect search request.
2. The record of staff #1 indicated that the staff member has lived outside of Virginia within the
last 5 years.
3. Staff #3 stated that they have lived outside of Virginia about 3.5 years ago.
Plan of Correction: As soon as I get the email link to file for out of state, I will finish all background checks on [staff #3] - then email back to [licensing inspector].
