During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 5/20/2025. The on-site inspection began at 1:40 pm and ended at 2:30 pm. The inspector reviewed compliance in the areas listed above. There were 5 children present and 1 caregiver. The inspector reviewed 5 children?s records and 2 caregiver/household member records on-site. This inspection included document review, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Standard 22.1-289.035-A
The center is required to have employees undergo a background check every five years. The most recent central registry check for caregiver/household member #1 was overdue by 5 months. The most recent central registry check for caregiver/household member #2 was overdue by 5 months.
Plan of Correction: Will get them done immediately.
Standard 8VAC20-800-180-A
A tuberculosis (TB) screening is required every two years from the last screening. An updated TB screening for Caregiver/Household Member #2 was not completed and was overdue by two months.
Plan of Correction: Will be done with in 2 weeks.
Standard 8VAC20-800-210-A
Caregivers shall obtain a minimum of 16 clock hours of training annually. Caregiver #1 did not have documentation of completing the 16 hours of training.
Plan of Correction: Will get them done.
Standard 8VAC20-800-510-C
Occupied cribs, cots, rest mats, and beds shall be at least 12 inches from each other. There was less than approximately 5" of space between 4 of the cots that were occupied by children.
Plan of Correction: Will separate cots immediately.
Standard 8VAC20-800-730-D
Medication, including refrigerated medication and medications for caregivers and household members, shall be kept in a locked place using a safe locking method that prevents access by children. There were two insulin injecting pens sitting on the dining table. The barrier gate was open which did not prevent access to the children that were awake and walking around.
