Inspector Notes

An unannounced inspection was initiated on 3/18/2026 and completed on 3/19/2026 in response to a complaint received by the licensing office on 2/26/2026 relating to allegations of supervision . The inspector reviewed 1 caregiver record on-site. This inspection included document review and interviews.

The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.

Standard 8VAC20-790-250-F
Caregivers who work directly with children shall annually attend the department's health and safety update course. Caregiver #1 had not completed this annual training in the past year.

Standard 8VAC20-800-120-B
Assistants' and substitute providers' records shall contain documentation of the education and training as required. The file for Caregiver #1 did not include documentation of education and programmatic experience.

(Repeat Violation) An assistant who is left alone with children in care shall have current certification in cardiopulmonary resuscitation (CPR). The certification for Caregiver #1 expired two months ago. Caregiver #1 was left alone with the children.

(Repeat Violation) A tuberculosis (TB) screening is required every two years from the last screening. An updated TB screening for Caregiver #1 was not completed and was overdue by one year.

In addition to satisfactory completion of first aid training and CPR training, caregivers shall obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities. Caregiver #1 did not have sixteen hours of annual training in the past year.
