Inspector Notes
An unannounced focused monitoring inspection was conducted in the center 3:50 pm -5:20 pm on August 9, 2023 and concluded on August 17, 2023. At the time of entrance, 36 children were in care with 6 staff members present. The sample size consisted of 3 children?s records and 5 staff records. Children were observed playing in centers, reading stores and dancing.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, observation and staff interviews, the licensee did not ensure there shall be in each grouping of children at least one staff member who meets the qualifications of a program leader or program director.
Evidence: The following staff were designated as program lead teachers and their records do not include documentation of the required training/ education required to meet the qualifications of a program lead or program director.
1.Staff #1, the designated program lead staff in the Infant Room was observed as one of two staff supervising 5 infants. Center Director confirmed there was not a staff member in the Infant Room that met qualification for program lead.
2.Staff #5, the designated program lead staff in the Schoolage Room was observed as the only staff supervising 12 Schoolage children. Center Director confirmed Staff #5 did not meet qualification for program lead.
#2 Staff #5 was paired with a qualified teacher /Assistant Director for remainder of day. Her last day was on 8/0/2023. Also paired with Assistant Director . A qualified lead teacher was paired with teachers to correct supervision of children .
Based on observation and staff interview, the licensee did not ensure the center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions. This list shall be dated and kept confidential in each room or area where children are present.
Evidence: The current written list of all children's allergies, sensitivities, and dietary restrictions was not dated.
Based on record review and staff interviews, the licensee did not ensure there was a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
Evidence: The record for child # 1 has a diagnosed food allergy (ham) and the record did not include a written care plan, to include instructions from a physician.
