Inspector Notes
This inspection was conducted by licensing staff both on-site and using a remote protocol, including telephone contacts and interviews.
A complaint inspection was received by the department on September 29, 2021. An on-site complaint inspection was conducted on October 27, 2021. A complaint incident was received by the department regarding allegations in the areas of hand washing and toileting procedures. The inspector reviewed compliance in the areas and conducted interviews.
The evidence gathered during the investigation supported the allegation of non-compliance with standards or law and violations were issued.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

2Evidence: 1. Staff 1 stated she puts gloves on prior to changing a diaper and washes hands after the diaper change.
2. Staff 2 stated she puts gloves on prior to changing a diaper and washes hands after the diaper change.
3. An outside witness stated she puts gloves on prior to changing a diaper and then washes hands after the diaper change.
Evidence: 1. Staff 1 stated the diapering table is sprayed with bleach water and then wiped after 10 seconds.
2. Staff 2 stated the diapering table is sprayed with bleach water and then wiped after 1 minute.
3. Staff 3 stated the diapering table is sprayed with bleach water and then wiped immediately.
4. An outside witness stated she used baby wipes to wipe the diapering table after a diaper change.
