During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was conducted on June 24, 2025. The on-site inspection began at 11:30 a.m.and ended at 2:30 p.m. The inspector reviewed compliance in the areas listed above. There were 116 children present and 21 staff. The inspector reviewed 10 children?s records and 10 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-570-B
Bottles shall not be used while the child is in his designated sleeping location. In the twos room, 2 children were observed with bottles while lying down on their cot, the designated sleeping location.
Plan of Correction: The bottles were removed from the children laying on their cots.
Standard 8VAC20-780-80-A
Repeat violation: For each group of children, the center shall maintain a written record of daily
attendance that documents the arrival and departure of each child in care as it
occurs. At approximately 11:45 a.m. on 6/24/2025, there were 14 children present in the twos room, but only 11 were marked present on the attendance record. There were 14 children present in the older 2s room, but only 12 were marked present on the attendance record.
Plan of Correction: The attendance was corrected on site. Staff will be retrained on maintaining attendance.
