Inspector Notes
An unannounced inspection was conducted on September 23, 2025 in response to a complaint received by the licensing office on August 29, 2025 relating to allegations of supervision. This inspection included a tour of the facility, interviews, and observations.
The preponderance of evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to injury reporting, and are documented on the violation notice issued to the program.

The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. An injury report for a serious injury that occurred on site on 8/26/2025 could not be located at the center for the inspector on 9/23/2025.
Plan of Correction: Injury reports will be documented and kept on site.
Standard 8VAC20-780-80-B
The center shall inform the department's representative as soon as practicable, but not to exceed two business days, of any injury to a child that occurs while the child is under the supervision of the center and requires outside medical attention. On 8/26/2025, a child sustained an injury on the playground and received outside medical attention. The center did not report the injury to the Department as required.
Plan of Correction: Injuries that require outside medical treatment will be reported to the Department within 2 days as required.
