Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on April 28, 2021 and concluded on May 18, 2021. The director was contacted by telephone to initiate the inspection. There were zero children present and one staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed two children's records and two staff records submitted by the facility to ensure documentation was complete.
The information gathered during the inspection determined that there were violations with applicable standards or law. Violations were documented on the Notice of Violation emailed to the provider.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

1. The record for Child #1 did not contain name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached.
Evidence:
1. The record for Staff #2 contained a request for a search of the central registry, dated 02/25/2021, but no completed central registry form. The date of hire for Staff #2 was 03/02/2021. There was no documentation of contacting the Office of Background Investigations regarding the status of the central registry request.
Evidence:
1. Fingerprints for Staff #2 were dated 03/03/2021, which was after the documented date of hire, 03/02/2021.
