Inspector Notes

An unannounced renewal inspection was conducted on 10/20/2022. There were 55 children, ages 5 year-11 years, and 8 staff members were present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, physical plant, and staffing and supervision. The children were observed in the following: arriving to the program, during afternoon snack and in indoor and outdoor free choice activities. A total of 5 children?s records, 6 staff records and 5 board officer/agent records were reviewed. The inspector arrived for the inspection at 2:10 pm and departed at 4:00 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on observation, the center failed to ensure that the areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

Evidence:
1. The metal seals around a pipe under the sink in the girl?s restroom were rusted and sharp. The pipe was exposed and accessible to the children.
2. There was a hole in the wall of the boy?s restroom.
3. There was a metal plate on the outside wall of the facility that was sharp. The metal plate was accessible to the children.
