Inspector Notes

An unannounced monitoring inspection was conducted on 10/23/2023. There were 73 children, ages 5 year-12 years, 7 staff members and various volunteers were present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, physical plant, and staffing and supervision. The children were observed in the following: arriving to the program, during afternoon snack, in indoor and outdoor free choice activities, and during homework. A total of 7 children?s records and 6 staff records were reviewed. The inspector arrived for the inspection at 2:00 pm and departed at 4:30 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on record review, the center failed to ensure that staff shall submit documentation of a negative tuberculosis screening at least every two years from the date of the initial screening or testing.

Evidence: The most recent tuberculosis screening in the record for Staff 2 was dated 10/14/2021; there was not a follow-up tuberculosis screening available for review which would have been required by 10/14/2023.
Plan of Correction: Will get a screening this week.
