Inspector Notes
An unannounced monitoring inspection was conducted on 03/13/2024. There were 67 children, ages 5 year-11 years, 6 staff members and various volunteers were present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, physical plant, and staffing and supervision. The children were observed in the following: arriving to the program, during afternoon snack, in indoor and outdoor free choice activities, and in small group activities. A total of 7 children?s records and 3 staff records were reviewed. The inspector discussed the arrival process, supervision, and outdoor play spaces with the staff. The inspector arrived for the inspection at 1:30 pm and departed at 4:10 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
Violations
4Evidence: The record for Child 1 did not contain documentation that Child 1 has received the required immunizations as required by the State Board of Health.
Evidence: The record for Child 3 did not contain a copy of the child?s physical examination.
Evidence: The most recent approval from the Health Department was dated 11/03/2022; an annual update was not obtained by 11/03/2023.
Evidence:
1.The record for Child 1 did not contain the first date of attendance.
2. The record for Child 2 did not contain the following information: the name, address and phone number of two designated people to call in an emergency if a parent cannot be reached (the addresses for both designated people were incomplete); and the first date of attendance.
