Inspector Notes

An unannounced monitoring inspection was conducted on 12/11/2023 from 10:40am to 12:00pm. There were 6 children ages 10 months to age 4 years old (15 points) with the Provider and 1 assistant. The programming area, attendance, menus, Provider, Assistant and Household member records, 6 children?s records, emergency drills, injury reports, emergency supplies, and policies were reviewed. Children were observed participating in musical instruments, literacy activities, reading, sensory play and singing. Handwashing procedures were also observed. Areas of non-compliance are identified in the Violation Notice.
Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the Violation Notice, and then return a signed and dated copy to the licensing office by 5pm on 12/29/23. Please email me at angela.dudek@doe.virginia.gov with any questions.

Evidence: The record for Household Member #3 (Date of Move in 8/25/23), did not contain documentation of a completed Central Registry record check or Criminal History check from all states where they resided within the last five years completed by the end of the 30th day of beginning residence in the home.
Plan of Correction: I have requested for the criminal history.
