Inspector Notes

An unannounced complaint inspection was initiated on 09/07/2023 from 9:30am to 10:10am in response to a complaint regarding transportation and background checks. There were 9 children present (17 points) ages 6 months to 3 years old with the Provider and 1 assistant. Interviews were conducted with the provider, 2 child files and 2 household member files were reviewed. Based on the preponderance of evidence collected during the investigation, the complaint was determined to be not valid. There were 2 violations cited unrelated to the complaint. The exit interview was conducted on 9/7/23 with the Provider. Description: Based on review of the written attendance record and interview with the Provider, the Provider did not ensure they maintained a written record of children in attendance each day.

Evidence:
1)During the inspection in was determined there were 9 children present, but the written record reflected there were 5 children present.
Plan of Correction: Was taken care that day.
Standard 8VAC20-800-60-B
Based on review of 2 children?s records, the Provider did not obtain all of the required documentation for child records.

Evidence: The record for Child #1 did not contain the contact information for 2 people to call in an emergency if parents can?t be reached, the father?s work phone number, allergy information, parent acknowledgement of whether or not the Provider holds liability insurance, policy for the administration of medication to parents, or proof of child?s identity.
