Inspector Notes

An unannounced Renewal inspection was conducted on 11/17/2023 from 9:35am to 10:50am. There were 7 children ages 5 months to age 4 years old (17 points) with the Provider and an assistant. The programming area, attendance, outdoor play area, menus, Provider, Assistant and Household member records, 6 children?s records, emergency drills, emergency supplies, and policies were reviewed. There was no medication onsite. Children were observed participating in indoor riding toys, tv time and playing with toy cars. The Provider and Assistant had current certification in CPR and First Aid training. Areas of non-compliance are identified in the Violation Notice.

Based on review of 2 household records, the Provider did not obtain for each Household member or caregiver a current report of Tuberculosis screening every 2 years from the date of the first screening.

Evidence: The record for Caregiver #2 contained a Tuberculosis screening that was last completed on 10/19/21.

Based on documentation review, the provider did not ensure they maintain documentation of current rabies vaccination.

Evidence: There was no documentation on file that a dog who lives at the home was current with a rabies vaccination. The rabies vaccination on file expired on 3/13/23.
