Inspector Notes
An unannounced monitoring inspection was conducted on 05/08/2024 from 9:30am to 10:40am with the Provider. There were 11 children ages 8 months to age 4 years old (27 points) with the Provider and 2 assistants. The programming area, attendance, outdoor playground area, menus, Provider, Assistant and Household member records, 3 children?s records, emergency drills, emergency supplies, and policies were reviewed. Children were observed participating in napping and art. Handwashing procedures were also observed. There was an adequate number of staff present with current certification in CPR and First Aid training. Areas of non-compliance are identified in the Violation Notice.

Violations
5Evidence: The Provider had documentation that the last fireplace inspection was completed on 1/13/23 and stated that it had not been inspected since this date.
Evidence:
During the inspection in was determined there were 10 children present, but the written record reflected there were 8 children present.
Evidence: The brown plastic play structure had large cracks in the plastic on 2 sides that could pinch a child?s hand or fingers.
Evidence: During inspection, residue and debris were noticed on the diaper pad surface.
Evidence: During the inspection, the provider stated that they did not have a menu ready for the remainder of the current week.
