Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 9/12/24 and completed on 9/12/24. The on-site inspection began at 8:40am and ended at 9:30am. The inspector reviewed compliance in the areas listed above, There were 10 children present and 2 caregivers; the point total was 24. The inspector reviewed 3 children?s records and 2 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 9/23/24. A POC submitted after this date will not appear on the public website.

Standard 22.1-289.011-F
Based on observation and staff interview, the center did not ensure they post the findings of the most recent licensing inspection of the facility.

Evidence: The most recent licensing inspection of the facility was not posted.
Plan of Correction: Was confused to what need to be posted has discussed with inspector.
Standard 8VAC20-790-250-C
Based on observation, the Provider did not ensure that items tied across the top or corner of a crib or playpen or toys hung from the sides with strings or cords should be removed when the child begins to push up on hands and knees or is five months of age, whichever occurs first.

Evidence: Child #1 (age 5 months) was resting in a crib that had 2 toys hung down the sides of the crib from the top rails.
Plan of Correction: Toy (2) were removed. Infant does not pull up yet.
Standard 8VAC20-800-210-A
Based on review of 2 Caregiver records and Provider interview, the Provider did not ensure that all caregivers obtain a minimum of sixteen clock hours of training annually.

Evidence:
1)The record for Caregiver #1 did not contain documentation of the completion of any training hours in the last year. The Provider confirmed during inspection that there was no training to report.
2)The record for Caregiver #2 contained documentation of 10 hours of annual training instead of the required 16 hours.
Plan of Correction: Caregivers are now doing annual training.
Standard 8VAC20-800-50-A
(Repeat Violation) Based on review of the written attendance record and interview with the Provider, the Provider did not ensure they maintained a written record of children in attendance each day.

Evidence:
During the inspection in was determined there were 9 children present, but the written record reflected there were 6 children present.
Plan of Correction: All names were written on check in sheet.
Standard 8VAC20-800-530-F
Based on observation, the provider did not ensure that no soft bedding of any kind should be used under or around infants.

Evidence: Upon arrival to the home, a blanket was observed under infant #1 (age 5 months) on top of the fitted sheet while the infant was resting.
Plan of Correction: Blanket was removed.
Standard 8VAC20-800-920
(Repeat Violation) Based on documentation and provider interview, the provider did not have documentation of a menu for the current one-week period.

Evidence: During the inspection, the provider stated that they did not have a menu ready for 3 days of the current week.
Plan of Correction: Menu was completed.
