Annually each person in a supervisory position must be certified by a practicing physician or physician assistant to be free from any disability which would prevent him from caring for children under his supervision. Staff #5 certification was a month past due.
Plan of Correction: Staff #5 will make an appointment with physician to complete required paperwork.
Standard 22.1-289.031-A-8
A person trained and certified in first aid will be present at the child day center whenever children are present or at any other location in which children attending the child day center are present. No one on site during the inspection was certified in first aid.
Plan of Correction: Director will ensure that staff are trained in first aid.
Standard 22.1-289.031-B-4
Ensuring that all children in the center are in compliance with the provisions of ? 32.1-46 regarding the immunization of children against certain diseases. Child #1 had no documentation of immunizations in their record.
Plan of Correction: Staff will get the required immunization document for child #1.
