Inspector Notes
A Subsidy Health and Safety inspection was conducted on 2/27/2024 to review supplemental health and safety requirements. There were 9 children present with 3 staff supervising.
A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection.
Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Based on a review of five staff records and interview, the vendor did not ensure that one staff annually completed the department's health and safety update course as required.
Evidence:
1. The record of staff #3 contained a health and safety update certificate from 2022.
2. Administration acknowledged that staff #3 did not complete the 2023 health and safety update.
